Parents: Please fill in the Medical Administration Form and your preferred Alternate Emergency Contacts for your son or daughter by logging in to your Parent Account on Skyward Family Access.

6th Grade

The states require all 6th graders to have a documented physical. Please take a physical form to your child’s next well visit for the office to complete it, and submit the completed form, along with a copy of the immunizations, to the NURSE or MAIN OFFICE (who will see that the nurse receives it). PLEASE remember…the Tdap and MCV (meningitis) vaccines are due for the START of 7th grade. Please discuss this with your child’s physician at their 6th grade well visit, and be sure to send in a printout of any immunizations they receive for their school record.

*Scoliosis screenings are mandated by the state in 6th grade. This screening will take place in the phys ed class in the spring (April) If the nurse has not received a physical form by that time which has been completed by the physician indicating that this was checked at the well visit.

7th Grade

The states requires a documented dental exam in 7th grade. The completed form can be turned in anytime during the 7th grade school year. Please take a dental form to your child’s next dental exam and return the completed form to the NURSE or the MAIN OFFICE (who will see that the nurse receives it). If it is your desire to have your child examined by the school dentist (NO ACTUAL DENTAL WORK WOULD BE COMPLETED, JUST A DENTAL ASSESSMENT), please notify the school nurse as soon as possible.
*Proof of immunization of MCV and Tdap vaccines are required for 7th grade.

SCOLIOSIS SCREENINGS will be done for all 7th graders, as required by the State of Pennsylvania, during physical education classes in February.  Students who have turned in a physical form from their private physician or who have had a sports physical during the CURRENT school year will NOT be screened.

8th Grade

If you have not submitted a completed physical and/or dental exam form for your child while they have been a student at West, please do so to update their school records so that the nurse can pass along their most updated information as they transition to the high school next year. If you are unsure if you have submitted either a physical or a dental exam for your child, simply email or call Ellen Grossman (groell@wilsonsd.org) or 610.670.0180 ext. 4917) and that information can be quickly obtained.
