Dear Parent/Guardian –

Have you submitted a . . .

• Physical exam for your Kindergarten aged or 6th grade child?
• Dental Exam for your Kindergarten aged, 3rd grade, or 7th grade child?

If you can’t remember, please call Nurse Sweeney or Nurse Nadine at 610-670-0180 extension 1730 to find out.  These exams are mandated by the State of Pennsylvania.  We would greatly appreciate your help in sending them to us as soon as possible.

Both forms can be accessed from the WSD Heath Services website (www.wilsonsd.org > Departments > Health Services- Nurse) or more directly from Nurse Sweeney’s website.

For your convenience, the physician/dentist may fax the forms directly to our fax # 484-334-6445.
