Physical Exams for Athletics
Exams provided by the Wilson School District Physician are $20 (cash or check/money order payable to Wilson School District) and are held in the High School Nurses Suite. Please enter at the last canopy, along Grandview Boulevard.
If you are unable to attend a school physical exam session, or if you cannot get an exam from your private physician, contact a local urgent care facility. They will do physical exams for school activities. Physical exams for the 2020-2021 school year must be dated June 1, 2020 or later.
2020-2021 Sports Physical Examinations – NEW PROCEDURE – sports physicals will be offered by appointment only for the upcoming 2020-2021 school year and ALL student-athletes must wear a mask during their exam.
Below are the links for both high school sports and middle school sports. We will extend these hours, IF NEEDED, but not before all time slots have been filled. Please remember that you always have the option of going to your personal physician or a local urgent care facility.

ATHLETIC PHYSICAL INFORMATION:
All student-athletes attending PIAA schools are required to have a physical examination before participating in athletics. Since 2008, the PIAA has also required that student-athletes grades 7-12 must have their parent/guardian and their school physician or personal physician complete a form called the CIPPE (updated Oct 2020) (Comprehensive Initial Pre-Participation Physical Evaluation) as the official record of the physical examination for athletic physical examinations.
A portion of the CIPPE form (Sections 4, 7, 10) are used for re-certification for additional sports in which the student-athlete wishes to participate and another section is reserved for those participating in wrestling.
The CIPPE form is the ONLY physical exam form acceptable for participation in athletics.
Students are required to obtain a physical evaluation for a given school year after June 1 of the upcoming school year. The physical exam will remain valid through the end of the school year barring major injury or illness. (See below for major injury/illness information.)
There are ten sections to the CIPPE (updated Oct 2020):
- Sections 1-5 must be completed by the parent/guardian and student.
- Section 6 must be completed by a licensed physician (M.D. or D.O.).
- Section 7 is used for re-certification for additional activities or following major injury/illness.
- Section 8 is used by a physician for re-certification following major injury/illness.
- Section 9 is used only by those participating in wrestling.
- Section 10 has been added due to COVID-19.
All sections must be returned to the student’s school where it is kept on file.
RE-CERTIFICATION for Students for Winter & Spring Seasons (w /NO major injury / illness)
Students who have participated in the fall season who wish to participate in the winter and/or spring seasons do not require a new physical exam. However, a student and the parent/guardian must complete a re-certification for each sport in which the student-athlete wishes to participate. Barring any major injury/illness since the physical exam, simply complete Sections 4, 7 & 10 of the RECERT updated Oct 2020 forms and return them to your school.
RE-CERTIFICATION for Students for Winter & Spring Seasons (w/ major injury/illness)
In the event the student has suffered a major injury or illness since the last physical exam:
- Section 7 must be completed by the parent/guardian AND
- Section 8 must be completed by a licensed physician (M.D. or D.O.)
