Notification for students with food allergies are submitted to the building nurse. The nurse communicates the allergy information to Food Services. If a student has a disability that requires substitutions or modifications in the school meals program, a “Medical Plan of Care for School Food Services” form must be completed and signed by a licensed physician and returned to the Food Service Coordinator. This form is available by contacting the Food Service Office at 610-670-0180, ext. 1147 or online at www.wilsonsd.org (Food Service page). If you have filled out the “Medical Plan of Care for School Food Serivces” form in the past and the information remains unchanged, simply email firstname.lastname@example.org stating the name of the child, grade, and the fact that the information is unchanged. We will keep a copy of this update attached to the original form to verify that nothing has changed. If there has been a change to your child’s allergy condition, a new form will need to be filled out and signed by a licensed physician.
